Condition posttraumatic deforming arthrosis stage
Study type case series
N / Groupings posttraumatic deforming arthrosis stage l - lll (260) / hip joint (72), knee joint (84), ankle joint (40), shoulder joint (14), elbow joint (35), wrist joint (15) (187 men + 73 women, aged 24-73) [part of a total group of 1100 patients with musculoskeletal conditions]
Type of acupuncture LA for stage l patients; LILT for stage ll and lll patients (LA if pain persistent)
Points used general restorative points: P-6, SJ-5, LI-4, LI-10, ST-5, Du-14; segmental and local points; points specific for bone (BL-11), connective tissue (SP-5), muscle (GB-34); other BAPs - hip joint: GB-29, GB-30, GB-31, GB-39, ‘ci-chun’ (?), jiankua (N-LE-55)(?); knee joint: BL-40, GB-39, LIV-8, ST-35, ST-36, SP-5, SP-9; ankle joint BL-60, BL-62, KI-6, GB-39, GB-40, GB-41, GB-42, LIV-2, LIV-3, ST-36, SP-5; shoulder joint: Sl-10, SJ-5, LI-4, LI-11, LI-14; elbow joint: SI-8, P-6, SJ-5, LU-5, LI-4, LI-10, LI-11, LI-12; wrist joint P-6, P-7, P-8, SJ-5, LI-4 [acupts used in all stages, with local joint irradiation in stages II-III; if pain persistent, auricular points used in stages II-III too]
Parameters used For joint irradiation: 632.8 nm, 30 mW, flux density 20 mW/cm2 [HeNe LG75]; for BAPs: body points - 632.8 nm, 30 mW, flux density 20 mW/cm2; ear points: 2 mW/cm2 [HeNe LG75 + ELA apparatus APL-01 ‘Diagnost-2’].
Stimulation duration joints: 30-60 secs/field (3-5 fields/joint), total 5 mins; BAPs: 5-6 secs/pt, 3-5 mins/session
Number of sessions daily sessions (joints 20-25; BAPs 15-20)
Follow up 1st: 6-10 mths; 2nd: 1.5-2 yrs
Endpoint measure clinical, X-ray and laboratory examinations, haemodynamic and biochemical findings, computerised remote thermography
Outcome normalisation of microcirculation and exchange processes; in 88.4% of patients, no pain or decreased pain at end of treatment, with increased range of motion of affected joint and improved walking; no effect in 8.5% patients, with no pain reduction in 3.1% of patients; in 35% of patients, pains (but lighter) recommenced after 1st follow up, while 65% of patients experienced a recurrence-free period of 1.5-2 yrs
Additional notes study demonstrates analgesic and anti-inflammatory effect of HeNe laser; in some patients, deterioration of sleep after 5-6 sessions, general weakness, increase of arterial pressure by 20-30 mm; After 2-3 days break from treatment, these occurences stopped
